6/30/2023 EnchantaVaughn Approved Behavior: Anthony displayed appropriate behaviors at the start of the session. Anthony implemented good listening skills. Anthony is aware that the sessions are for him to deescalate unwanted feelings and to challenge himself to overcome symptoms of ADHD. Anthony was observed following directions and complying to positive reinforcement that was provided by counselor. Anthony displayed excellent transition. Anthony was observed in the session displaying good behaviors and expressing a positive attitude. Anthony was supported for acknowledging his mistakes and managing his anxiety on his own without relying on everyone else. Behavior: Anthony transitioned into the next activity without conflict and proceeded to engage into meaningful conversation. Anthony expressed positive thoughts. Anthony was respectful of staff and presenting favorable actions, at this time. Anthony was observed asking and answering appropriate questions. Anthony exhibited excellent communication skills by being polite and patient creating a positive working atmosphere. Anthony was relieved to know that he wasn’t alone and that many boys his age go through the same things. Anthony appears to be functioning well in his crisis situations due to the increased support and supervision that he has received today. Intervention: QMHP-C monitored Anthony individualized behavior modifications in collaboration with his siblings and guardian. QMHP-C reflected with Anthony on his day, reviewing both good and bad experiences. QMHP-C provided Anthony with encouragement to demonstrate the positive behaviors and expectations discussed. QMHP-C applauded Anthony for being a leader. QMHP-C provided simple non-verbal praises as Anthony was observed displaying positive behaviors. QMHP-C reinforced appropriate coping skills in the home. QMHP-C processed Anthony positive responses and challenged Anthony to maintain appropriate behaviors throughout the entire week. Intervention: QMHP-C inquired about at home activities. QMHP-C motivated Anthony to respond to exercise and express his feelings. QMHP-C offered active listening skills. QMHP-C will collaborate with mother regarding behaviors and performance when a counselor isn’t present. QMHP-C modeled language for Anthony to use while in the home and in the community that will promote pro-social behaviors and effective communication skills. QMHP-C encouraged Anthony to communicate to his mother when he needs help in the home, at school, or out in the community. QMHP-C applauded Anthony for his commitment and efforts with doing his best and having a great turn around after yesterday’s episode. Response: Anthony smiled and hugged QMHP-C for being there for him when he needed. Anthony displayed positive interaction with his peers and was supported with building healthy relationships at home. Anthony implemented the positive reinforcement provided by his mother and counselor which allowed him to remain calm and active. Anthony expresses good statements about his day saying he was doing his work, using his manners, and being fair to others. Anthony is more confident, as evidenced by assertive and proud tone. Anthony have been doing a better job controlling impulsive and hyperactive behaviors. Response: Anthony transitions smoothly to the next activity, displaying appropriate behaviors. Anthony knows what is expected of him and is trying to make better choices. Anthony verbalizes positive feelings about himself but recognizes areas he needs to work on. Although Anthony has difficulty staying focused, he is extremely dedicated to getting his work and chores done and helping his mom around the house. Anthony presents a positive attitude when given the opportunity to share his opinion on important matters concerning his safety. Anthony was able to stay motivated and enjoy his session, as observed.
